All 50 States Submitted Applications for Their Share of the $50 Billion Rural Health Transformation Program Funding
States are both the applicants and the direct recipients of these funds. To maintain funding, each state must demonstrate satisfactory progress toward its identified goals and comply with all program terms and conditions.
Review your state’s application to understand how your organization can access available funds. Aligning your institutional goals with state priorities will help maximize clinical outcomes, value-based payment opportunities, and compliance.
After the Application
States will partner with Federally Qualified Health Centers (FQHCs), Community Health Centers (CHCs), primary care clinics, high schools, community-based organizations, rural health clinics, Critical Access Hospitals, and clinics supporting Opioid Use Disorder/Substance Use Disorder (OUD/SUD) services. Through these partnerships, states aim to advance goals such as:
-
- Strengthening public-private partnerships
- Increasing access to services
- Improving healthcare delivery
- Reducing chronic disease
- Addressing unmet OUD/SUD needs
- Supporting rural providers in serving their communities
